Colleen Mae Formanek

Colleen A. Formanek, 87 of Onalaska, WI, passed away on Thursday, July 7, 2016 at Hillview Healthcare in La Crosse. She was born in La Crosse, WI, on April 16, 1929 to Leslie and Nora (Johnson) Adams. She married Thomas A. Formanek in Onalaska on June 12, 1948 and he preceded her in death in 2008. Colleen is survived by her children; Dawn Formanek and Randy Formanek both of La Crosse; her three grandchildren Mitchell, Shaylee and Tanner and two sisters, Marilyn (Joe) Tovar of Onalaska and Jane (Larry) Mosher of Viroqua, WI. In addition to her husband Tom, she was preceded in death by six siblings.
Memorial services will be held at 1:00pm on Friday, July 15, 2016 in the chapel at Oak Grove Cemetery. Pastor Duane Baardseth will officiate. Burial will follow. Friends may call on the family at the chapel on Friday from 12:30pm until the time of services. The Schumacher-Kish Funeral and Cremation Services of La Crosse is assisting the family.
